Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:       Building 1:  On or about December 12, 2013 employees were exposed to a greater than 6 foot fall while installing metal roofing on a 6:12 pitch roof of a three story apartment complex under new construction.

29 CFR 1926.602(d): The employer did not certify that each operator had been trained and evaluated as required by paragraph 29 CFR 1910.178(l):       Building 1:  On or about December 12, 2013, an employee was operating a forklift to lift metal roofing up to the roof of a three story apartment complex building and the employer did not certify or evaluate the training.
