Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.     Jobsite: The employer did not ensure that each employee who was exposed fall hazards of up to 33 feet 10 inches above the ground while re-roofing a flat roof was protected from the hazard of falling by the use of appropriate fall protection devices and/or systems.
