29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    (a) Cutoff Saw Area:  On or about December 10, 2013 and at times prior, employees were exposed to unexpected energization during maintenance operations such as but not limited to, replacing saw blades or clearing jammed lumber from the saw.    (b) Peeler Areas: On or about December 10, 2013 and at times prior, employees were exposed to unexpected energization during maintenance operations such as but not limited to, clearing jammed lumber from the peelers.    (c) Green Chain Area:  On or about December 10, 2013 and at times prior, employees were exposed to unexpected energization during maintenance operations such as but not limited to, clearing lumber from the conveyor.

The rotating shaft was approximately 12 inches in length and approximately six feet from the ground.
