29 CFR 1926.403(b)(2): Listed, labeled, or certified equipment was not installed and/or was not used in accordance with instructions included in the listing, labeling, or certification: (a) Madison Place at Crystal Pond Rd Southborough, MA:On or about December 4, 2013, two employees were observed using a Defiant GFCI 3 way splitter that was plugged into an extension cord, which is contrary to the instructions printed on the device, and therefore not in accordance with this subpart.
