OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:    105 N. Main St., Wakefield, NE 68784 (Old Tanker Bay):  Employees were exposed to physical harm such as amputation hazards when loading a tanker. The employer did not ensure that employees used cotter pins to prevent the rear valve from closing, which is known to be a amputation hazard. An employee had the tip of their right ring finger amputated when he attempted to pull the rear valve out on a tanker when the valve suddenly closed on or about 10/03/2013.          AMONG OTHER METHODS, FEASIBLE AND ACCEPTABLE ABATEMENT METHODS TO CORRECT THIS HAZARD ARE:          1)    Train employees to recognize the dangers of working with the rear valve of the tanker without the insertion of a cotter pin or other safety retainer device.       2)    Develop work rules and disciplinary procedures prohibiting the loading of tankers without a cotter pin or other safety retainer device.      3)    Utilize cotter pins or other safety retainer device and administrative controls that provide the same or a greater level of protection for the employees who load tankers.      Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed.
