29 CFR 1926.20(a)(1):  The contractor or subcontractor for any part of the contract work did not require any laborer or mechanic employed in the performance of the contract to work in surroundings that were not unsanitary, hazardous, or dangerous to his health or safety:    (a) Canopy, South Side: On December 11, 2013, the controlling contractor did not exercise reasonable care to prevent employee(s) who were not equipped with fall protection while working on a sloped canopy, installing Prolinx Building Wrap to the exterior of the building, from being exposed to a fall of approximately 9 feet and up to 13 feet from the ground below.    (b) Canopy, South Side: On December 11, 2013, the controlling contractor did not exercise reasonable care to prevent employee(s) who utilized a  20-foot Werner Extension Ladder with improperly positioned foot pads and with side rails that did not extend at least 3 feet above the upper landing to which the employees were gaining access, from being exposed to a fall of approximately 9 feet from the ground below.    (c) Canopy, South Side: On December 11, 2013, the controlling contractor did not exercise reasonable care to prevent employee(s) who utilized a Husky 8-foot step ladder in a leaning position while working on a sloped canopy, installing Prolinx Building Wrap to the exterior of the building, from being exposed to a fall of approximately 9 feet and up to 13 feet from the ground below.
