29 CFR 1910.1030(c)(1)(ii): The employer's Exposure Control Plan did not contain at least the elements in 29 CFR 1910.1030(c)(1)(ii)(A) through (c)(1)(ii)(C):    a) Assisting in dental procedures and/or handling contaminated instruments from those procedures present a likelihood of skin, eye, mucous membrane or parenteral contact with blood.  The generic exposure control plan for bloodborne pathogens within a compliance seminar manual kept in the dentist's office had not been adapted to the practice through the inclusion of site-specific details including a list of jobs / tasks with occupational exposure, as well as the schedule & method of implementation of program elements such as Hepatitis B vaccination, employee training, and recordkeeping.     Condition(s) were noted on or about 12/13/2013.

Upon initial hire, and at least annually thereafter, each employee with such occupational exposure was not provided training including, but not limited to; access to a copy of the OSHA standard on bloodborne Pathogens and explanation of the workplace exposure control plan, modes of transmission of bloodborne pathogens, methods established for preventing or reducing exposure, and procedures for reporting an exposure incident.    Condition(s) were noted on or about 12/13/2013.
