29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces (NOTE: A sign reading DANGER-PERMIT-REQUIRED CONFINED SPACE, DO NOT ENTER or using other similar language would satisfy the requirement for a sign):      (a) On or about December 12, 2013, and at certain times before, employees performing sanitation and other duties in the elevator pits were exposed to potential confined space hazards in that the employer failed to install appropriate signage designating the elevator pits as confined spaces.      Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.    NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the techniques to be utilized for the control of hazardous energy:       (a) On or about December 12, 2013, and at certain times before, employees in the Sanitation Department entered elevator pits without isolating the kinetic (gravitational) energy posed by the elevator cars above them.  The relevant energy control procedure did not require the placement of pipe stands in the elevator pit to arrest any unexpected downward movement of the elevator car.       Abatement certification is required for this Item.  Use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET for this purpose.     NOTE:  IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM.  FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):      (a) On or about December 12, 2013, employees in and around the control room for the 10,000 pound elevator were exposed to electrical shock hazards in that an electrical cabinet door was standing open with exposed energized parts.  The door latch had been damaged preventing it from securing the door in its closed position.      Abatement certification for this Item is not required.  Corrected during inspection.
