29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift:  a.  On or about December 11, 2013, on the east side of the building, employees working from an aerial lift were not wearing harnesses and lanyards, exposing themselves to a 14 feet fall hazard.

On or about December 11, 2013, on a low slope roof, employees were not protected from a 14 feet fall hazard by the use of a fall protection system.
