29 CFR 1910.133(a)(1): Protective eye equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment:   a) Throughout facility: Goggles were not provided and required for use for environmental services employees required to use corrosive chemicals, such as, but not limited to, 23 TB Disinfectant and Deodorizer and 65 Disinfecting Heavy Duty Acid Bathroom Cleaner, for cleaning purposes, on or about 12/13/13.   NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  a) Kitchen: An eyewash station, capable of 15 minutes of continuous flushing, was not provided where employees used corrosive materials, including, but not limited to, Ecolab Greasestrip Plus and Ecolab Lime Away, for cleaning purposes, on or about12/13/13.  b) Throughout facility: Eyewash stations, capable of 15 minutes of continous flushing, were not provided where employees used spray bottles containing corrosive chemicals, such as, but not limited to, 23 TB Disinfectant and Deodorizer and 65 Disinfecting Heavy Duty Acid Bathroom Cleaner, for cleaning purposes, on or about 12/13/13.    NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1910.1030(f)(1)(ii)(D): The employer did not ensure that all medical evaluations or procedures including the hepatitis B vaccine, vaccination series, post-exposure evaluation, or follow-up, including prophylaxis, were provided according to recommendations of the US. Public Health Service current at the time these evaluations and procedure took place:  a) Throughout facility: The employer did not have proper post exposure treatment services set up as required by the US Public Health Serivce in that the employer's contracted medical service provider did not utlitze rapid HIV testing when processing source individual's blood, on or about 12/13/13.  NOTE: THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT CERTIFICATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
