29 CFR 1910.134(c): The employer did not develop and implement a written respiratory protection program with required worksite-specific procedures and elements for required respirator use:  (a)  At the facility, employees are required to use a North 7700, half-face respirator with organic vapor cartridges when applyihg tetrachloride to parts to be vacuum coated.  (b) At the facility, employees are required to use a 3M half-face respirator with HEPA filters when bead blasting with the enclosure door open.

(b)  At the facility, employees required to use a 3M half-face respirator with HEPA filters had not received a medical evaluation.
