29 CFR 1926.52(b): When employees were subjected to sound levels exceeding those listed in Table D-2 of this section, feasible administrative or engineering controls were not utilized:  An employee using a jackhammer during demolition of bank vault walls was exposed to continuous noise levels at  972.5% of the permissable daily noise exposure, (8-hour time-weighted average sound level of 90 dBA), which is 9.72 times the allowable exposure limit.  The equivalent sound level of the measured dose is 106.4 dBA.  Sampling was performed for 254 minutes on December 13, 2013 with the unsampled portion of time, 226 minutes, considered to represent zero for the exposure calculation.  Engineering controls to include but not limited to hammer mufflers and silencers were not used during jackhammering operations.  Violation occurred on or about 12-13-13
