29 CFR 1926.502(d)(21): Personal fall arrest systems shall be inspected prior to each use for wear, damage and other deterioration, and defective components shall be removed from service:    On or about December 15, 2013, two welders working approximately 60 feet above the ground were exposed to a fall due to the anchor point for their personal fall arrest system being installed and used on an unstable column.
