29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    Employees engaged in recycling operations were exposed to potential eye hazards from flying particles when operating the can crusher, the baler and other equipment and the employer did not enforce the use of protective eyewear.

29 CFR 1910.305(b)(2)(i): Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose:    Employees engaged in recycling operations were exposed to electrical hazards because the following boxes were not provided with covers:        a)  The junction box above the bailer was not covered, thus exposing live conductors.      b)  The box on the front of the bailer motor was not covered, thus exposing live conductors when the machine was operational.      c)  The breaker box in the scale room was not provided with a cover, thus exposing live conductors.

29 CFR 1910.305(c)(4): Snap switches mounted in boxes did not have faceplates installed so as to completely cover the opening and seat against the finished surface:  Employees engaged in recycling operations in the warehouse were exposed to electrical hazards because the faceplates on the light switches were not attached and not installed flush to seat against the finished surface.
