McMahon dba Beals-McMahon Painting, 2 East 3rd Avenue, Warren, PA, 16365-on or about November 25, 2013, and times prior thereto: Company employees voluntarily used 3M brand half mask respirators with organic vapor cartridges (#6001) while working next to drywall buffing operations. Appendix D of 29 CFR 1910.134 had not been provided to the respirator users.

McMahon dba Beals-McMahon Painting, 2 East 3rd Avenue, Warren, PA, 16365-on or about November 25, 2013, and times prior thereto: Company employees voluntarily used 3M brand half mask respirators with organic vapor cartridges (#6001) while working next to drywall buffing operations. The employee's respirator was not maintained in a sanitary condition as evidence by the fact that paint residues were observed on the inside of the facepiece of the employee's half mask respirator.
