29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.:    a)  The rotating gearbox shaft of the G2 Blue Extruder did not have a guard to protect workers from contact with the rotating shaft as they stand in between the pellet tote and the G2 Blue Extruder to vacuum pellets from the bottom of the tote.    b)  The adjustable guards of the two EZ Rider, model number 1650-03, card attachment machines were affixed in the elevated position which not adequately protect the operator from the attachment needle point of operation.

29 CFR 1910.1200(f)(6)(ii)  Except as provided in 29 CFR 1910.1200(f)(7) and 29 CFR 1910.1200(f)(8), the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the product identifier and words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals and which, in conjunction with the other information immediately available to employees under the hazard communication program, would provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:    a)  In the Glue area, five application containers of Cyanoacrylate Adhesive which are used daily were not labeled with identity or hazard warning.  b)  In the Glue area, one spay bottle container of Quick Tac 2 Accelerator was not labeled with identity or hazard warning.
