29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:   a) Left Side, Front of Structure - A 24' extendsion ladder was used to gain access to the roof, approximately 30' above ground, extended approximately 12" past roof landing on or about 12/16/13.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) Top Center Roof of Structure - Workers  finalizing the replacement of the roof, affixing ridge vent caps, were not protected from falling approximately 30' to the ground and scrap material below, on or about 12/16/13.    David's Contracting was previously cited for a violation of this occupational safety and health standard or it's equivalent (1926.501(b)(13)) which was contained in OSHA inspection number 660878, citation number 1, item number 1 and was affirmed as a final order on 12/10/2012, with respect to a workplace located at 5833 Potomac Avenue NW, Washington DC 20010.
