29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   On or about December 11, 2013, employees were exposed to rotating parts from the aluminum/steel lathe machine, in that the rotating head of the lathe was not covered or guarded to prevent rotating parts or flying chips/sparks from contacting the operator.

29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o):   On or about December 11, 2013, employees were exposed to serious caught-in, crushing and or struck-by injuries, in that there was a pulley that did not have a guard that fully enclosed the moving parts on all exposed sides.

29 CFR 1910.305(d)(2):   Panelboard enclosures. Panelboards shall be mounted in cabinets, cutout boxes, or enclosures designed for the purpose and shall be dead front. However, panelboards other than the dead front externally-operable type are permitted where accessible only to qualified persons.  On or about December 11, 2013, employees in the assembly and grinding rooms were exposed to hazard of electrocution from open areas within an electrical panel which did not provide a protection from inadvertent contact with exposed live electrical connections.

29 CFR 1910.305(g)(2)(iii):  Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:     On or about December 11, 2013, a 120V flexible cable had pulled loose from its attachment point to a portable outlet box leaving the cable attached to the device by its internal conductors.
