29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: On or about December 17, 2013 at the above address site, an employee was exposed to a fall hazard of approximately 40 feet while performing electrical work from the basket of a truck mounted aerial lift and not wearing a safety harness with a lanyard attached to the boom or basket of that lift.
