On or about December 16, 2013, at the above addressed jobsite, an employee was exposed to the hazard of being struck-by a conveyor table located on the trailer of a flatbed truck that was not supported or secured to prevent it from tipping over while the anchor straps were being ratcheted down around the conveyor table.     NOTE: ABATEMENT CERTIFICATION AND DOCUMENTATION IS REQUIRED FOR THIS VIOLATION.
