29 CFR 1910.132(d)(1)(i): When the employer had assessed the workplace hazard(s) and determined that hazard(s) were present, the employer did not select and/or have each affected employee use the types of personal protective equipment that would protect the affected employee from the hazard(s) identified: Jobsite - Refinery Lab: the employer did not ensure that the workplace hazard assessment was done for employees cleaning Mettler melting point cups, pouring chloroform and/or cleaning chloroform spills, where chemical splashing hazards were present or likely to be present. These employees were not provided AND/OR required to wear the proper gloves. There was an incident (on September 27th, 2013) where an employee sustained an eye injury after a chloroform splashing accident. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citations.
