Employees of a subcontractor were performing welding activities at sulfuric acid plant #7 where explosive gases were being generated upstream.  The accummulating gases created an explosive atmosphere which were ignited by the welding task and resulted in an explosion, on or about 11/23/2013.

The welding was being performed on duct work which had not been provided a vent to allow potentially explosive gases to safely escape to the atmosphere.  An explosion occured while welding activities were being conducted, on or about 11/23/2013.
