29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section.     Throughout the facility - On or about December 17, 2013, procedures were not developed, documented and utilized for the control of potentially hazardous energy, thereby exposing employees to the unexpected re-energization of machinery.    a. Tool room - On or about December 17, 2013, the employer did not provide LOTO procedures for the upright band saw in the tool room, model Metal Master, serial number 413233, thereby by exposing employees to the hazards associated with the unexpected re-energization of machinery.     b. Tool room - On or about December 17, 2013, the employer did not provide LOTO procedures for the Rockford lathe, model 96Hx16, serial number not present, thereby by exposing employees to the hazards associated with the unexpected re-energization of machinery.     c. Machine shop - On or about December 17, 2013, the employer did not provide LOTO procedures for the Milling Machine, model H, serial number 18-4998, thereby by exposing employees to the hazards associated with the unexpected re-energization of machinery.

1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.     Throughout facility - On or about December 03, 2013, the employer had not performed periodic inspections to ensure that documented lockout / tag out procedures were being followed by authorized employees.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.    a. Tool room - On or about December 17, 2013, the employer did not properly guard the exposed unused portion of blade on the upright band saw in the tool room, model Metal Master, serial number 413233, thereby by exposing employees to the hazards associated with exposed cutting surfaces and rotating parts.    b. Tool room - On or about December 17, 2013, the employer did not properly the rotating parts on the Rockford lathe, model 96Hx16, serial number not present, thereby by exposing employees to the hazards associated with rotating parts.    c. Machine shop - On or about December 17, 2013, the employer did not properly guard the rotating parts on the Milling Machine, model H, serial number 18-4998, thereby by exposing employees to the hazards associated with rotating parts

29 CFR 1910.212(a)(3)(ii): The point of operation of machines, whose operation exposes an employee to injury, shall be guarded. The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.  Production Department - On or about December 17, 2013, the employer did not have the points of operations completely guarded, in that the Chicago Riveter(s), Serial number(s) 99-114, 912-814, and 55-1045, were not guarded, thereby exposing employees to the hazards associated with points of operations.
