29 CFR 1910.132(f)(1): The employer did not provide training to each employee who is required by this section to use PPE:  a) Facility: On or about December 17, 2013: The employer failed to train employees in accordance with 29 CFR 1910.132(f)(1)(i-v) who are required to use fall protection while using the Snorkelift to perform maintenance activities in the facility at heights of between 15 and 20 feet.

29 CFR 1910.1200(f)(9): The employer did not ensure that labels or other forms of warning were legible, in English, and prominently displayed on the container, or readily available in the work area throughout each work shift:  (a) Outside rear area of the facility: On or about December 17, 2013: The employer failed to ensure that the propane storage tank for the propane used to power the forklifts in the facility was labeled as to what the tank contained.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:   (a) Facility: On or about December 17, 2013: The employer failed to ensure that all employees were trained on the new format for safety data sheets, Global Harmonization System pictograms, new hazard rating system and warning words.
