29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) Letter Shop: Employees were exposed to electrical hazards, such as, but not limited to, the metal box with prepunched knockouts was being used as a pendant for the Booklet Maker as the metal electrical box is listed or labeled to be attached to a wall or piece of equipment. (b) Letter Shop: Employees were exposed to electrical hazards as the metal box with prepunched knockouts was being used as a pendant for the Perfect Binder as the metal electrical box with prepunched knockouts is listed or labeled to be attached to a wall or piece of equipment. (c) Letter Shop: Employees were exposed to electrical hazards as the metal box with prepunched knockouts was being used as a pendant for Inserter # 1 as the metal electrical box with prepunched knockouts is listed or labeled to be attached to a wall or piece of equipment.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: (a) Letter Shop: Employees were exposed to electrical hazards as the flexible cord for the Booklet Maker, running to the metal box with prepunched knockouts, did not have adequate strain relief as the outer insulation had pulled back next to the box exposing interior wiring. (b) Letter Shop: Employees were exposed to electrical hazards as the flexible cord for the Perfect Binder that was plugged into the metal outlet box did not have adequate strain relief as the outer insulation to the plug had pulled back exposing interior wiring. (c) Letter Shop: Employees were exposed to electrical hazards as the flexible cord running to the four plex outlet box for the Inserter # 1 did not have adequate strain relief as the outer insulation for the electrical cord had pulled back exposing interior wiring.
