At this commercial building under construction, an employee worked from a Genie, S-40, aerial lift without tying a lanyard from a body belt or harness to the boom or personnel platform. The employee was exposed to falls from the platform from heights greater than 10 feet.

The employee worked in close proximity to the unprotected sides and edges and was exposed to falls of approximately 25 feet.
