29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) Right side roof: Employees without any means of fall protection were engaged in framing activities being exposed to a fall of approximately 25 feet, on or about 12/3/2013.   b) Front of building: Employees were using a single plank unguarded walkway to access the worksite being exposed to a fall of approximately 8 feet, on or about 12/3/2013.

29 CFR 1926.1053(a)(2): Ladder rungs, cleats, and/or steps were not parallel, level, and uniformly spaced when the ladder was in position for use:    a) Exterior wall right side: Employees were accessing the second floor with a job made ladder that did not have parallel and uniformly spaced rungs, being exposed to a fall of approximately 10 feet, on or about 12/3/2013.
