29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c):    East end of the dam base:  On or about December 17, 2013, and times prior to, employees were installing a drainage pipe in a trench at the dam base.  No protective system was in use in the portions of trench 5 feet and deeper.  This exposed the employees working in the trench to the hazard of being caught in a cave in.
