29 CFR 1910.146(c)(1): The employer did not evaluate the workplace to determine if any spaces were permit-required confined spaces:  (a)  In the drying area:  On December 17, 2013 and at times prior to the employer exposed employees to confined space hazards in that the employer has allowed employees to enter a confined space such as but not limited to dryers for maintenance and cleaning without first evaluating the space to determine if it is a permit required confined space.   (b)  In the Tunnel Washer area:  On December 17, 2013 and at times prior to the employer exposed employees to confined space hazards in that the employer has allowed employees to enter a confined space such as but not limited to the wash tunnel for maintenance without first evaluating the space to determine if it a permit required confined space.

29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:   (a)  Facility wide: On December 18, 2013 and at times prior to; the employer failed to establish an energy control program for servicing each machine or equipment where sources of stored energy could cause physical injury or death.

29 CFR 1910.157(g)(2): The educational program to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided to all employees upon initial employment, and at least annually thereafter:  (a)  Facility wide:  On December 18, 2013 and at times prior to the employer failed to provide fire extinguisher training to employees that are required to extinguish incipient stage fires as required by this regulation for new hires and other employees.
