29 CFR 1910.212(a)(2):   Guard(s) on machine(s) were not affixed to the machine or secured elsewhere when attachment to the machine was not possible:    Work Site located at 402 N. 3rd Street, Norfolk, NE; Processing Area:  An Reitz Extruder machine (grinder) was fitted with a guard which was not secured.  The guard serves two purposes; cover a mixing vat and guard the discharge point of the extruder.  The discharge point of the extruder has a face plate which serves as a shear for the extruder auger.      On 11-18-2013, a temporary employee sustained amputation injuries to the left ring and middle fingers as a result of removing the guard prior to ensuring the machine was off.  The guard was easily removed and not secured to the vat or the extruder.       Abatement certification is required for this violation.  The abatement certification sheet is enclosed with the citations.
