Employees were exposed to fall hazards of 12.8 feet and greater while working and walking across scaffolding platforms with missing guardrails.      a) Employees were standing/working off of scaffold platforms 12.8 feet high with no guardrails along the exterior edge of the scaffold.   b) Employees were working off of scaffolding 16 feet high erected in between two block walls. No fall protection/guardrails was present and being used.

Spruce St., Tampa, FL, as observed on or about 12/19/2013, employees were working around and exposed to uncapped, protruding rebar:    a) Employees were exposed to uncapped rebar extending out of the ground from concrete caissons measured to be 3ft. and 2in. high.  b) Next to the new rebar retrieval pile, there were rebar pieces sticking out of the ground approximately 2-3 feet that employees could fall into.   c) Where concrete/form work was being done in the excavated section of the jobsite, uncapped rebar approximately 2-3 feet high was present.
