29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  (a)  On the rooftop of the Berrien Nursing Center, on or about December 19, 2013, no fall arrest system or other forms of fall protection were in use by employees who were working while standing on the roof (pitch approximately 4:12), resulting in an approximately 10-foot fall hazard.
