Employees on the work room floor were exposed to an amputation hazard as a result of a Makita chopsaw, used to cut tile material, which did not have the bottom guard mechanism designed for such purpose in place.  Abatement note: Abatement certification is required for this item use the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET  for this purpose.
