29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: On or about December 18, 2012, an employee plugged in the portable reciprocating saw where the end of the cord was not equipped with strain relief and the insulated wires were exposed, exposing the employee to an electrical shock hazard.

29 CFR 1926.451(a)(6): Scaffolds were not designed by a qualified person and were not constructed and loaded in accordance with that design. On or about December 18, 2013, the employees constructed the pump jack scaffolding that was not designed, constructed and loaded by a qualified person. The warning plate states "do not use above 30 feet". The scaffold was used up to approximately 40 feet high, exposing employees to the hazard of scaffold collapse and falls.
