29 CFR 1910.1030(d)(3)(ix): Gloves were not worn when it was reasonably anticipated that the employee might have hand contact with blood or other potentially infectious materials, mucous membranes, or non-intact skin:  On or about December 19, 2013, and times prior thereto, at the above addressed jobsite, the employer did not ensure employees in the pre-pack department were using gloves when handling instruments coming out of washer unit when it was documented that instruments were found to have blood on them after coming out of the wash cycle.

29 CFR 1904.8(a): Work-related needlestick injuries or cuts from sharp objects contaminated with another person's blood or potentially infectious material were not entered on the OSHA 300 Log as a privacy case.  On or about December 19, 2013, the employer at the above addressed jobsite did not record a needlestick injury as a privacy case on the OSHA 300 log 2013, the log displayed employees name and possible exposure to Hepatitis C patient.

29 CFR 1910.1200(h)(3)(iv):   The details of the hazard communication program developed by the employer, did not include an explanation of the labels received on shipped containers and the workplace labeling system used by their employer; the safety data sheet, including the order of information and how employee could obtain and use the appropriate hazard information:  On or about December 19, 2013, at the above addressed jobsite, the employer did not provide adequate training or information regarding the hazards of chemicals, according to the Globally Harmonized System of Classification and Labeling of Chemicals with pictograms or representations as a part of the new SDS and label format.
