29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structure members were not installed between the top edge of the guardrail system and the walking/working surface when there was no wall or parapet wall at least 21 inches (53 cm) high: (a) Second Story Loft - on or about December 20, 2012 - the guardail system was missing a midrail. Opening was 22 feet wide. Elevation to the first story floor was approximately 10 feet, two inches.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a) Second Story Loft - on or about December 20, 2012 - the employer did not provide a comprehensive fall protection training program to the employees that were performing framing installations.
