29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition:     On or about December 10, 2013 at the Darling International facility- employees were exposed to serious injuries in that slip, trip and fall hazards were created by the abundance of clutter and debris throughout the maintainance stockroom.

29 CFR 1910.253(b)(2)(ii): Assigned storage spaces for cylinders was not located away from elevators, stairs, or gangways where cylinders could not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons:  On or about December 10, 2013 at Darling International facility:  Employees were exposed to an oxygen cylinder that was not chained or stored properly. 29 CFR 1910.303(b)(1)(iv): Electric equipment was not free from recognized hazards that were likely to cause serious physical harm to employees, such as damaged electrical insulation:   On or about December 10, 2013 at the Darling International Facility- employees were exposed to serious injuries due to electrical shock, fire, or burns, in that flexible cords on a Rotary Skillsaw were found to be in disrepair with damaged or missing outer insulation, abrasions, and splices made with electrical tape.               Abatement certification is not required for this item.

29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:     On or about December 10, 2013 at the Darling International Facility- employees were exposed to serious injuries, in that the employer had not marked and identified the breakers in the electrical panel located in the Maintainance Stock room of the warehouse.    Abatement certification is not required for this item.

29 CFR 1910.303(g)(1):   Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:   On or about December 10, 2013 at the Darling International Facility- one or more workers were exposed to hazardous conditions while working in the shop floor area due to an electrical panel being blocked.   Abatement certification for this item is not required.

29 CFR 1910.305(g)(2)(iii):  Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:      On or about December 10, 2013 at the Darling International Facility- employees were exposded to a 220V flexible cable had pulled loose from its attachment point to a portable outlet box leaving the cable attached to the device by its internal conductors.   Abatement certification is not required for this item.
