29 CFR 1910.147(d): The established procedures for the application of energy control (the lock-out or tag-out procedures) did not cover the actions listed and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): Dicing Area - On or about December 13, 2013, the employer did not ensure that lock-out/tag-out devices were applied to isolate energy sources on the Dicer Machine #2102804, when employees were performing servicing and maintenance tasks. Employee received a laceration on the top of his right hand by the energized equipment while performing servicing and maintenance.
