29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person prior to the start of work, and as needed throughout the shift, for evidence of situations that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions. 200 Terminal Dr., Lafayette, LA: On or about October 23, 2013, the employer failed to protect employees from potential cave in hazards associated with trenching and excavations when the employer failed to meet the requirements of having a competent person inspect the trench/excavation and the protective systems prior to employees entering the trench and as needed throughout the shift.

29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: 200 Terminal Dr., Lafayette, LA: On or about October 23, 2013, three employees were observed working in a trench over 5 feet deep without an adequate protective system. The employees were digging around a large pipe and box to remove the box and replace with a new one. Employees were exposed to hazards associated with trench collapse/cave-in.
