29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: (a) On or about 12/19/2013, employees were not provided safe access and egress in a trench excavation with an approximate depth of 15 feet. Employees utilized a built in ladder on the trench box which was not maintained free of slipping hazards, and walked the length of the top to access a ramp built in the ground.
