Atlantic Fabrication and Boiler, Shop: On or about 12/16/13, a Ryobi bench grinder serial no. W032509609 equipped with two 8" abrasive wheels was not provided with tongue guards during the grinding of tungsten rods, thereby exposed employees to the potential of flying fragments.

29 CFR 1915.134(b): Floor and bench mounted grinders shall be provided with work rests which are rigidly supported and readily adjustable. Such work rests shall be kept a distance not to exceed 1/8 inch from the surface of the wheel. a.) Atlantic Fabrication and Boiler, Shop: On or about 12/16/13, the work rest on the 8" Ryobi bench grinder serial no. W032509609 was not kept at a distance not to exceed 1/8" from the surface of the wheel in that the work rest was approximately 1" from the surface of the wheel, thereby exposed employee to a laceration hazard.
