29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: (a) On December 12, 2013, at a job site located at 4427 Park Spring Court, Lee's Summit MO., employees were exposed to serious fall hazard approximately 12 to 17 feet above a lower level without any means of fall protection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: (a) On or about December 12, 2013, at job site 4427 Park Spring Court, Lee's Summit, MO, an employee was exposed to a fall hazard as he accessed a roof using a ladder that was less than 3 feet above the upper landing surface.
