29 CFR 1910.119(j)(5): The employer did not correct deficiencies in equipment that were outside acceptable limits (defined by the process safety information on paragraph (d) of this section) before use:  a.   On or about, December 4, 2013, the stock flow differential pressure meter, a sensor input to the chlorine dioxide block valve interlock, was not operational, and the interlock had been bypassed, exposing employees to a chlorine dioxide gas release.

On or about December 4, 2013, in the bleach plant, a written procedure was not implemented for temporary operations with the differential pressure meter interlock to the chlorine dioxide block valve in bypass.

29 CFR 1910.120(c)(5): Personal protective equipment was not provided and used during initial site entry in accordance with 29 CFR 1910.120(c)(5)(i) through (c)(5)(iv):    a.  On or about December 4, 2013, in the bleach plant, employees who responded to a chlorine dioxide gas release did not wear respirators during the initial response.
