29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: On or about 12/10/2013, during the storm water renovation project at N Lois Avenue (between Hillsborough Avenue and W Crest) in Tampa: A) No ladder was observed at pipe junction box S-277. B) No ladder was observed at pipe junction box S-273. C) The ladders that were later provided at pipe junction boxes S-273 and S-273 did not extend 3 feet above the landing.
