29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides: (i) persons can pass, (ii) there is moving machinery, or (iii) there is equipment with which falling materials could create a hazard:    On or about December 23, 2013, in the facility, a storage mezzanine platform, about 8' feet above ground level, was not fully guarded by a standard railing, or equivalent, on all open sides, thus exposing employees performing fabrication operations to a fall hazard.

29 CFR 1910.23(d)(1)(iii): Every flight of stairs having four or more risers shall be equipped with standard stair railings or standard handrails, as specified: On stairways less than 44 inches wide having both sides open, one stair railing on each side:  On or about December 23, 2013, in the facility, the wooden stairway (ten-riser, 42" wide) leading to the approximate 8' high storage mezzanine did not have standard stair railings or handrails on each open side, thus exposing employees performing fabrication operations to a fall hazard.

29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage shall be separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour:    On or about December 23, 2013, in the facility, oxygen cylinders were stored adjacent to fuel-gas (e.g. acetylene) cylinders and not separated a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one-half hour.  As such, employees performing fabrication operations in the area were exposed to a fire hazard.
