29 CFR 1910.305(g)(1)(iv):  Flexibles cords and cables were used for purposes prohibited by subparagraphs (a) through (e) of this paragraph:       a)  On or about December 19, 2013, and at times prior, employees were exposed to electrical shock and fire hazards because flexible cords were attached to building surfaces and used as a substitute for fixed wiring (such as conduit) in the fabrication shop.  Flexible cords were attached to the wall  with metal staples and direct wired into outlets and fixtures.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws:                                                                   a)  On or about December 19, 2013, and at times prior, employees were exposed to electrical shock and fire hazards because electrical cords near the old saw had  strained and damaged insulation at the joint where the cord entered the plug.
