29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:     A)    Truck Sales & Service, Inc., located at 100 Cleveland Road, Norwalk, OH 44857: On or about December 23, 2013, the employer failed to establish a lockout/tagout program for the control of the hazardous energy, which includes procedures, training, and periodic inspections, while performing activities such as, but not limited to, changing drill bits on the equipment such as, but not limited to, the Central Machinery 16-Speed Heavy Duty Drill Press (Model CII7F, Serial # 152824) located in the facilitys maintenance area. Employees were exposed to amputation hazards while changing drill bits.    No additional abatement information is needed for this item.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    A)    Truck Sales & Service, Inc., located at 100 Cleveland Road, Norwalk, OH 44857: On or about December 23, 2013, the employer failed to ensure that the Central Machinery 16-Speed Heavy Duty Drill Press (Model CII7F, Serial # 152824), located in the facility's maintenance area, was properly guarded in order to prevent employees from inadvertently being caught by the rotating chuck during the drill press' operation. 29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch:    A)    Truck Sales & Service, Inc., located at 100 Cleveland Road, Norwalk, OH 44857: On or about December 23, 2013, the employer did not ensure that the right side of the Black & Decker 8" Heavy-Duty Bench Grinder (Type & Serial # G 2961343), located in the facility's maintenance area, was equipped with a work rest that was adjusted to 1/8 inch away from the abrasive wheel. Employees that operated the grinder were exposed to injuries from potentially jammed work and/or to struck-by hazards in a case of the wheel breakage.    No additional abatement information is needed for this item.
