29 CFR 1910.132(d)(1)(i): The employer did not select and have each affected employee use, the types of personal protective equipment (PPE) that would protect the affected employee(s) from the hazards identified in the hazard assessment: a) On or about 12/21/2014 - Parker building, the employer did not ensure employees were wearing the required PPE while loading and unloading furnaces or when responding to emergency situations for equipment such as, but not limited to, the Allcase A4 endothermic gas furnace, S/N#BC-44447. Employees were not protected from burns by the use of personal protective equipment such as, gloves, safety glasses, face shields, and hard hats.
