29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes: On or about 12/23/2014 at the Path Academy Construction Project, employees were exposed to fall hazards of approximately 10 to 12 feet while working at, near, and next to holes in the floors that were not covered nor were any fall protection systems in use. Advanced Framing Systems, LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 315339226, citation number 01, item number 001b and was affirmed as a final order on 07/21/2011, with respect to a workplace located at 907 Federal Road in Brookfield, CT.
