29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed:  West. Press room. Web offset presses: on or about December 23, 2013, an authorized employee locking out gluer unit on the Heidelberg Harris M600 for removing a drive unit, or changing counters on Harris M100L was not inspected periodically (at least once per year) to ensure his level of proficiency in the hazardous energy control procedures implemented by the employer to ensure sufficient level of protection and compliance.  Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1910.178(l)(6): The certification did not include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation:  North. Shipping and receiving dept. CAT and Doosan propane-gas powered sit-down forklifts: on December 23, 2013 employer did not retain required and adequate training and evaluation certificates for authorized forklift operators.  There is no abatement certification or documentation required for this item.
